I’m really happy you found the information useful. The EIN is best got by phone, and the W8BEN can be downloaded from either the IRS website or from Amazon.
Amazon’s help page is https://kdp.amazon.com/self-publishing/help?topicId=A1VDYJ32T5D3U4 – It contains links to all relevant forms.
The Amazon address you need to send your completed W8BEN to is at the bottom of the page.
